About the Role

Position Summary:

Agape Comfort Care Solutions is seeking a compassionate and experienced Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team as a PRN Case Manager. In this role, you will play a crucial part in ensuring the delivery of high-quality, client-centered care by overseeing service plans, conducting assessments, performing supervisory visits, and completing all necessary reports in accordance with Georgia state regulations.

Responsibilities:

Initial Client Assessments: Conduct comprehensive assessments of new clients, gathering information about their medical history, current needs, and desired outcomes. Develop individualized care plans that address their specific needs and preferences.
Service Plan Oversight: Oversee and manage client service plans, ensuring they are up-to-date, comprehensive, and aligned with the client's goals. Collaborate with the care team to make any necessary adjustments to the plan based on the client's evolving needs.
Supervisory Visits: Conduct regular supervisory visits to client homes to assess the quality of care being provided, ensure adherence to the care plan, and address any concerns or issues that may arise.
Documentation and Reporting: Maintain accurate and timely documentation of client assessments, care plans, supervisory visits, and other relevant information. Complete all required reports in compliance with Georgia state regulations.
Communication and Collaboration: Maintain open and effective communication with clients, families, caregivers, and other members of the care team. Collaborate with healthcare professionals and community resources to ensure coordinated and comprehensive care for clients.
Quality Assurance: Participate in quality assurance activities to continuously improve the delivery of care and ensure that services meet or exceed regulatory standards.
Other Duties: Perform other related duties as assigned by the Clinical Director.

Requirements

  • Licensure: Current and unrestricted LPN or RN license in the state of Georgia.

  • Experience: Minimum of 2 years of clinical experience in home health or a related field. Experience with geriatric populations and/or individuals with chronic conditions is preferred.

  • Knowledge: Thorough understanding of Georgia state regulations governing home care services. Familiarity with medical terminology, disease processes, and medications commonly used by the elderly.

  • Skills: Excellent assessment, communication, and interpersonal skills. Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines. Proficient in electronic health record (EHR) systems.

  • Other: Valid driver's license, reliable transportation, and willingness to travel to client homes throughout the service area.
